The rule is that if the regel comes from the MIDDLE, if the gug is a bit wide on either side, proportionately it's usually kosher. Fix by adding ink by widening the regel CAREFULLY so as not to make as small daled or backwards daled (which is also problematic).If the gug is really wide, then it's a problem even if it comes out from the middle.I think Rav Stern discusses all these scenarios in detail in Mishnas Hasofer.This case pictured is redeemable IMHO
